The 111th Attack Wing Legal Office offers free legal assistance on personal, civil legal matters to Guardsmen, retirees and their dependents. Some examples of personal, civil legal matters include: wills, powers of attorney, domestic relations, and Servicemembers Civil Relief Act (SCRA) matters.

Use this link to request powers of attorney, advance medical directives (also known as a "living will"), health care powers of attorney (who talks to your doctor when you are in a prolonged incapacitated state), durable powers of attorney (who pays your bills while in a coma) or a last will and testament.
After completing the questionnaire, you will be given a ticket number. Please email that number along with the last name you used to register the document to the 111th Attack Wing's JAG office
so the legal team can retrieve the information necessary to draft your documents. When we have completed your document, we will contact you to arrange a convenient appointment time to sign it.
